One Screw Loose

Sometimes the biggest tragedies stem from the silliest of problems. In an article published by the United Steel Workers, an accident occurred where a crane operator was unable to control the crane while moving a 28,000 lbs roll in a temper mill. In an otherwise routine operation, a catastrophic failure in the control system occurred. While one employee saved a friend with his heroics, he lost his own life in the process.
